    The Dirac equation in spherically symmetric fields is separated in two different tetrad frames. One is the standard cartesian (fixed) frame and the second one is the diagonal (rotating) frame. After separating variables in the Dirac equation in spherical coordinates, and solving the corresponding eingenvalues equations associated with the angular operators, we obtain that the spinor solution in the rotating frame can be expressed in terms of Jacobi polynomials, and it is related to the standard spherical harmonics, which are the basis solution of the angular momentum in the Cartesian tetrad, by a similarity transformation.Comment: 13 pages,CPT-94/P.3027,late

    Atrial Fibrillation and the Role of Thumb ECGs

    Atrial fibrillation (AF) may be underdiagnosed, and there is much that remains unknown about this prevalent and potentially life-threatening arrhythmia. AF epidemiology has been thwarted in part by the fact that about a third of patients with AF have no symptoms, those with symptoms may experience them intermittently or have vague symptoms, and it can be challenging to capture an episode on a 12-lead ECG, which is required for diagnosis. There are many significant knowledge gaps in our understanding of AF etiology and progression. A new user-friendly device that allows for frequent self-monitoring of the heart rhythm has been introduced. With the thumb ECG, patients can record a tracing multiple times a day. A smartphone app will soon allow them to interact with their healthcare providers about these ECG recordings. An ECG parser will allow for an algorithm-directed, rapid, automatic interpretation of these recordings with high specificity and sensitivity. This may help researchers learn more about the so-called silent AF, AF progression (and possible remission), and risk factors for AF. This technology holds great promise for patient care as well as for research into AF

    Deuterium Exchange Dynamics in Zr_2NiD_(4.8) Studied by ^2H MAS NMR Spectroscopy

    Variable temperature (VT) ^2H magic angle spinning (MAS) spectroscopy was employed to measure deuterium diffusion behavior in the Zr_2NiD_(4.8) phase. ^2H MAS NMR spectrum at ∼190 K provides with well-resolved 4 different site occupancies which can be assigned based on the crystal structure (16k (Zr_2Ni_2), 32m (Zr_3Ni), Zr_4 (16/ and 4b)). As the temperature rises, the ^2H peaks sensitively reflect the exchange behavior among the sites with evident change at around 230 K and reaching a uniform distribution of site occupancies, indistinguishable in NMR timescale, above 245 K. This behavior is reflected by the collapse of the ^2H MAS spectrum into a single peak. From analyses of VT MAS NMR spectra, we were able to extract multiple hopping rates and activation energies among face sharing interstices: for example, 32m ↔ 16/ hopping shows _(τc)=2.8×10^(-4)s at 245 K and E_a = 62.2 kJ/mol

    FMM-Head: Enhancing Autoencoder-based ECG anomaly detection with prior knowledge

    Detecting anomalies in electrocardiogram data is crucial to identifying deviations from normal heartbeat patterns and providing timely intervention to at-risk patients. Various AutoEncoder models (AE) have been proposed to tackle the anomaly detection task with ML. However, these models do not consider the specific patterns of ECG leads and are unexplainable black boxes. In contrast, we replace the decoding part of the AE with a reconstruction head (namely, FMM-Head) based on prior knowledge of the ECG shape. Our model consistently achieves higher anomaly detection capabilities than state-of-the-art models, up to 0.31 increase in area under the ROC curve (AUROC), with as little as half the original model size and explainable extracted features. The processing time of our model is four orders of magnitude lower than solving an optimization problem to obtain the same parameters, thus making it suitable for real-time ECG parameters extraction and anomaly detection.Comment: 23 pages, 14 figure

    The current practice of intra-aortic balloon counterpulsation: results from the Benchmark Registry

    OBJECTIVES This study presents clinical data from the first large registry of aortic counterpulsation, a computerized database that incorporates prospectively gathered data on indications for intra-aortic balloon counterpulsation (IABP) use, patient demographics, concomitant medication and in-hospital outcomes and complications. BACKGROUND The intra-aortic balloon pump (IABP) is widely used to provide circulatory support for patients experiencing hemodynamic instability due to myocardial infarction, cardiogenic shock, or in very high risk patients undergoing angioplasty or coronary artery bypass grafting. METHODS Between June 1996 and August 2000, 203 hospitals worldwide (90% U.S., 10% non-U.S.) collected 16,909 patient case records (68.8% men, 31.2% women; mean age 65.9 ± 11.7 years). RESULTS The most frequent indications for use of IABP were as follows: to provide hemodynamic support during or after cardiac catheterization (20.6%), cardiogenic shock (18.8%), weaning from cardiopulmonary bypass (16.1%), preoperative use in high risk patients (13.0%) and refractory unstable angina (12.3%). Major IABP complications (major limb ischemia, severe bleeding, balloon leak, death directly due to IABP insertion or failure) occurred in 2.6% of cases; in-hospital mortality was 21.2% (11.6% with the balloon in place). Female gender, high age and peripheral vascular disease were independent predictors of a serious complication. CONCLUSIONS This registry provides a useful tool for monitoring the evolving practice of IABP. In the modern-day practice of IABP, complication rates are generally low, although in-hospital mortality remains high. There is an increased risk of major complications in women, older patients and patients with peripheral vascular disease

    High expression of 5-lipoxygenase in normal and malignant mantle zone B lymphocytes

    <p>Abstract</p> <p>Background</p> <p>Human B lymphocytes can produce leukotriene B<sub>4 </sub>but the biological function of the 5-lipoxygenase (5-LO) pathway in B cells is unclear. In order to better understand and define the role of 5-LO in B cells, we investigated the expression of 5-LO mRNA and protein in subsets of B cells from human tonsils and different types of B cell lymphoma.</p> <p>Results</p> <p>Based on RT-PCR and western blot/immunohistochemical staining, with a polyclonal antibody raised against 5-LO, high expression of 5-LO was found in mantle zone B cells from tonsils. By contrast, only a weak expression of 5-LO was detected in germinal centre cells and no expression in plasma cells from tonsils. This pattern of 5-LO expression was preserved in malignant lymphoma with high expression in mantle B cell lymphoma (MCL) and weak or no expression in follicular lymphoma. Primary leukemized MCL, so called B-prolymphocytic leukaemia cells, and MCL cell lines also expressed 5-LO and readily produced LTB<sub>4 </sub>after activation.</p> <p>Conclusion</p> <p>The present report demonstrates the expression of 5-LO mainly in normal and malignant mantle zone B cells while the expression is low or absent in germinal centre B cells and plasma cells, indicating a role of the 5-LO pathway in B cells before the cells finally differentiate to plasma cells.</p

    Photobiomodulation Therapy is Able to Modulate PGE2 Levels in Patients With Chronic Non‐Specific Low Back Pain: A Randomized Placebo‐Controlled Trial

    Background and Objectives Non-specific low back pain (LBP) is responsible for triggering increased biomarkers levels. In this way, photobiomodulation therapy (PBMT) may be an interesting alternative to treat these patients. One of the possible biological mechanisms of PBMT involved to decrease pain intensity in patients with musculoskeletal disorders is modulation of the inflammatory mediators’ levels. The aim of this study was to evaluate the effects of PBMT compared with placebo on inflammatory mediators’ levels and pain intensity in patients with chronic non-specific LBP. Study Design/Materials and Methods A prospectively registered, randomized triple-blinded (volunteers, therapists, and assessors), placebo-controlled trial was performed. Eighteen patients with chronic non-specific LBP were recruited and treated with a single session of active PBMT or placebo PBMT. The primary outcome of the study was serum prostaglandin E2 levels and the secondary outcomes were tumor necrosis factor-α, interleukin-6 levels, and pain intensity. All outcomes were measured before and after 15 minutes of treatment session. Results PBMT was able to decrease prostaglandin E2 levels at post-treatment compared with placebo, with a mean difference of −1470 pg/ml, 95% confidence interval −2906 to −33.67 in patients with LBP. There was no difference between groups in the other measured outcomes. Patients did not report any adverse events. Conclusion Our results suggest that PBMT was able to modulate prostaglandin E2 levels, indicating that this may be one of the mechanisms involved in the analgesic effects of PBMT in patients with LBP.publishedVersio

    Photobiomodulation therapy does not decrease pain and disability in people with non-specific low back pain: a systematic review

    Question: In people with non-specific low back pain (LBP), what are the effects of photobiomodulation therapy (PBMT) on pain, disability and other outcomes when compared with no intervention, sham PBMT and other treatments, and when used as an adjunct to other treatments? Design: Systematic review of randomised trials with meta-analysis. Participants: People with acute/subacute or chronic non-specific LBP. Interventions: Any type of PBMT (laser class I, II and III and light-emitting diodes) compared with no treatment, sham PBMT and other types of treatment, or used as an adjunct to another treatment. Outcome measures: Pain intensity, disability, overall improvement, quality of life, work absence and adverse effects. Results: Twelve randomised controlled trials were included (pooled n = 1,046). Most trials had low risk of bias. Compared with sham PBMT, the effect of PBMT on pain and disability was clinically unimportant in people with acute/subacute or chronic LBP. In people with chronic LBP, there was no clinically important difference between the effect of PBMT and the effect of exercise on pain or disability. Although benefits were observed on some other outcomes, these estimates were imprecise and/or based on low-quality evidence. PBMT was estimated to reduce pain (MD −11.20, 95% CI −20.92 to −1.48) and disability (MD −11.90, 95% CI −17.37 to −6.43) more than ultrasound, but these confidence intervals showed important uncertainty about whether the differences in effect were worthwhile or trivial. Conversely, PBMT was estimated to reduce pain (MD 19.00, 95% CI 9.49 to 28.51) and disability (MD 17.40, 95% CI 8.60 to 26.20) less than Tecar (Energy Transfer Capacitive and Resistive) therapy, with marginal uncertainty that these differences in effect were worthwhile. Conclusion: Current evidence does not support the use of PBMT to decrease pain and disability in people with non-specific LBP.publishedVersio
